CHEM 474 Access Request Form

Complete this form to request an override to register for CHEM 474 (Biochemistry Laboratory) for Fall 2026 Registration. 

This form is primarily for BS/BA Biochemistry Major Declared & B.A.E. CHEM/BIOL Majors to share their CRN preferences. Outside majors seeking to enroll in CHEM 474, should also use this form. 

BS/BA Biochemistry and B.A.E. CHEM/BIOL applicants are prioritized first. Biochemistry applicants are then prioritized based on their Major (Phase II) admittance and Special Circumstances Request Form decisions. B.A.E. CHEM/BIOL applicants are prioritized based on their Special Circumstances Request Form decisions. All other applicants are prioritized after Biochemistry and B.A.E. CHEM/BIOL applicants by graduation date.
