Complete the below form to request a Major Evaluation for submission with your graduation application materials.

  • Please allow 2 weeks for your request to be processed.
  • If you have waited longer than 2 weeks, please send an email to chemistry@wwu.edu for a status update.

 

Upon completion, you will receive an email with your completed and signed evaluation attached.

  • Please be sure to use your WWU Student email address.  Submissions containing outside email addresses will not be processed.
