Special Circumstances Course Reservation Form

This form is for students that are Phase II Declared BAE majors and certain BS or BA majors within the Chemistry Department as described below.

All BAE Chemistry students should fill this form out the year before they intend to take CHEM 471, 472, 474, 461, or 462 so that we can reserve a seat. 

BA and BS Chemistry and Biochemistry students should only fill out this form if they are no longer in sequence with their admitted Phase II Cohort. This could be due to a course withdrawal, failing a course, not completing a prerequisite to remain with the cohort, or from not taking a cohort course for any other reason. Completing this form does not guarantee space within the next cohort. Cohort courses include: CHEM 434, 436, 461, 462, 463, 464, 465, 466, 467, 468, 471, 472, 473, and 474. 

Please note reservations should be requested, prior to the Phase II application for that quarter. For example, if you wish to take CHEM 461 Fall of 2021, then you should submit this form before the first day of Spring Quarter 2021, and if you want to take CHEM 461 in the Winter of 2022, then you should submit this form before the first Friday of Fall Quarter 2021. If your request is approved, that will be one less seat we can offer to a Phase II applicant. If you do not submit a request, there may not be a seat available.

If you only wish to take one or two courses within a series, please still select the series option. You may elaborate in the final question.

For example, if you wish to take CHEM 461 Fall of 2021, then you should submit this form before the first day of Spring Quarter 2021. If your request is approved, that will be one less seat we can offer to a Phase II applicant.
